PREPARING FOR YOUR CONSULTATION WITH DR TEH
Dr Teh provides a most comprehensive and state-of-the-art fertility service in conjunction with Melbourne IVF.
Please bring the following:
- A referral from your General Practitioner or family doctor
- All test results from your referring doctor e.g blood tests, ultrasound etc.
- A list of all medications that you are currently taking
- Any operation report or relevant specialist correspondence
- Medicare card and private hospital insurance information (if applicable)
Please arrive 10 minutes early to register for your Appointment. We recognise that your time is valuable, and make every effort to run on time. Occasionally, emergencies or patients require a little more time, we thank you for your patience and understanding. Should you require, interpreting services are also available.
If you need to cancel an appointment, please call the office during business hours (8am – 4:30pm) and allow at least 1 day’s notice so that we can offer your appointment time to other patients.
